- Currently receiving long-term opioid therapy (have an opioid prescription), OR

Under consideration for long-term opioid therapy
- A tool to help clinicians address important questions when treating chronic pain patients:
- Is long-term opioid therapy a reasonable option?
- Should you take extra precautions with this patient?
- What level of monitoring may be necessary to safely proceed with long-term opioid therapy?
